Part 2: Studio Policies & Financial Agreement

Enrollment & Tuition Terms

Semester Commitment: Enrollment requires a full semester commitment (Sem 1: Aug–Jan | Sem 2: Jan–June). Enrollment automatically rolls over to Semester 2 unless written notice is given.
Cancellation: Written notice must be provided at least 14 days before the next billing date to stop enrollment for the following semester.
No Refund Policy: All tuition payments are non-refundable. No refunds/credits for absences, vacations, or illness.
Annual Participation Fee: A mandatory $150 fee is due upon registration. This covers the annual program and recital costs for all students.
Discounts: A 5% sibling discount is available for families with multiple children.

Part 3: Attendance & Performances

Attendance & Rehearsals

Regular attendance is vital for progress and choreography.
Make-Up Classes: The studio is not obligated to provide make-up classes for student absences.
Performances: All students are part of the annual program. Participation in community productions and the end-of-year recital is a core part of the training experience.

I understand the attendance requirements and that my child is enrolled in a performance-based program.

Part 4: Legal Waivers

Medical & Liability Release

I hereby declare that there is no medical impediment to my child's participation. My child is under regular follow-up by a Physician and is qualified for the physical effort required for dance, acrobatics, and aerial activities. I agree to disclose any physical limitations or allergies to the management. I acknowledge the risk of physical injury and hereby release D Dance Spot Academy LA, its instructors, and staff from all liability for accidental injury or illness incurred during classes, rehearsals, or events.

I agree to the Medical Declaration and Release of Liability.

Media & Photo Release

I grant permission for images/video of my child captured during classes or events to be used solely for D Dance Spot LA promotional materials, social media, and publications. I waive any rights of compensation or ownership thereto.

I agree to the Photo and Media Release.

Part 5: Final Confirmation

By checking this box and submitting this form, I confirm that I have read, understood, and agree to be bound by all D Dance Spot Academy LA Studio Policies and the Enrollment Agreement.

Instagram Contest: Terms & Conditions

Eligibility:

Participants must be between the ages of 8 and 13 years old and provide parental consent (parents can submit from their account in representation of their children).

The contest is open to residents of Los Angeles County where permitted by law.

Employees and immediate family members of D Dance Studio are not eligible.

How to Enter:

1. Follow @ddancespot_la on Instagram.
2. ⁠Like the contest rules post.
3. ⁠Comment on the post with any dance emoji and tag 2 friends.
4. ⁠Share the contest post on Instagram Stories and tag us.
5. ⁠Optional for Extra Points:

Share a dance video or photo on your profile, use #DdanceIntoTheNewLocation, and tag us.

Contest Period:

The contest runs from January 16 to January 26th.

Selection of Winners:

Winners will be selected based on enthusiasm, creativity, and engagement.
Multiple (5) winners will be chosen and announced by the end of January.

Prizes:

Prizes include a free professional dance class or a gift card to use on any of our packages.

Winners will be contacted via Instagram Direct Message.
Publicity Release:

By entering, participants grant D Dance Studio permission to use their submitted content for promotional purposes.

General Terms:

This contest is in no way sponsored, endorsed, or administered by, or associated with Instagram.

Participants release Instagram from any responsibility for this contest.

Privacy:

Personal information collected will only be used for the purpose of this contest and will not be shared.
